These Appendices are available to you online. There is no charge to read, print, or download these valuable tools to your computer. It is the authors' hope that you will find these tools useful to you as a caregiver and that this information will help you to protect your elderly loved one -- or yourself -- from the common complications that affect the elderly.

NOTE: The appendices are available in two formats: Each appendix has a separate webpage that can be accessed using the hyperlinks on the right-hand side of the index below. This is the best format to use for those appendices where there are active hyperlinks created to direct you to an article or other resource on the Internet. (These are marked with an asterisk). Other appendices, designed to be used by the reader to create a portable medical record -- documents that can be printed, three-hole punched, and kept in a binder -- are available in Adobe PDF files that can be accessed and read using Adobe Acrobat Reader. See printing tips below.  Hyperlinks on the left-hand side of the index below will direct you to those documents. You will need to install the Adobe Acrobat Reader on your computer to read and print PDF files if you do not already have this program.  Adobe Acrobat Reader is free and may be accessed and downloaded here. PRINTER TIP: To Print PDF files in 8 1/2 by 11 inch format for insertion into your three-ring binder, open the PDF file, then click on the printer icon on the toolbar. On the pop-up menu, at Page Handling, choose "Fit to printer margins" on the pull-down menu.
